Subchorionic hematoma in pregnancy

Weeks 1–13
1st trimester
Subchorionic hematomas are most often identified during early ultrasound, sometimes after spotting and sometimes incidentally. The report may compare the collection with the gestational sac because relative size is more informative than a single volume estimate. Contact the obstetric team about any bleeding and keep the recommended repeat scan.
Weeks 14–27
2nd trimester
A collection that remains visible later in pregnancy needs an individualized ultrasound plan. Ask whether its size or location has changed and whether fetal growth or the placenta needs reassessment. New bleeding, fluid leakage, contractions, dizziness, or significant pain should not wait for the next routine visit.
Weeks 28–birth
3rd trimester
Third-trimester bleeding should be assessed urgently because causes other than a previously seen subchorionic hematoma may be responsible. Do not assume a known collection explains fresh bleeding. Go to obstetric triage for heavy bleeding, severe pain, faintness, contractions, fluid leakage, or reduced fetal movement.
What does the evidence show for subchorionic hematoma?
- Subchorionic hemorrhage is bleeding beneath the chorion that may cause light vaginal bleeding or be found incidentally. Ultrasound commonly shows a crescent-shaped collection. For a stable pregnant patient and fetus without major blood loss, conservative follow-up with repeat ultrasound is appropriate; evidence has not established routine bed rest as beneficial. 1
- In a retrospective series of 434 live singleton pregnancies with a subchorionic hematoma at 6 to 11 weeks, first-trimester loss was more frequent when the finding was identified at or before 7 weeks than after 8 weeks. Hematoma size as a fraction of the gestational sac correlated better with outcome than calculated volume. 2
- A retrospective cohort comparing 178 first-trimester subchorionic hematomas with 350 pregnancies without one found that large collections had higher frequencies of early loss, placental abruption, growth restriction, and preterm birth than controls and smaller collections. This association does not show that the hematoma caused those outcomes. 3
- In a propensity-matched cohort after fresh embryo transfer, an asymptomatic subchorionic hematoma at 6 to 8 weeks was not associated with statistically significant differences in live birth, miscarriage, or preterm birth. Within the hematoma group, vaginal bleeding was associated with lower live-birth frequency, showing why symptoms and population context matter. 4
- A meta-analysis of 16 singleton studies found no statistically significant overall association between first-trimester subchorionic hematoma and preterm delivery, while miscarriage associations differed between natural and assisted-conception subgroups. Heterogeneity among observational studies limits a single prognosis estimate for an individual pregnancy. 5
When should I call my provider about subchorionic hematoma?
Contact the obstetric team promptly for any pregnancy bleeding. Go to urgent obstetric assessment or an emergency department for heavy bleeding, severe abdominal or one-sided pelvic pain, shoulder pain, weakness, dizziness, fainting, regular contractions, fluid leakage, or reduced fetal movement. A known subchorionic hematoma does not rule out ectopic pregnancy, pregnancy loss, placental causes, or another source of bleeding. None of these lists replace your own judgment: if this symptom worries you, that on its own is enough reason to check in with your healthcare provider.

What does a subchorionic hematoma look like on ultrasound?

Does the size of a subchorionic hematoma matter?
Relative size can matter, but there is no single measurement that predicts an individual outcome. Research suggests the fraction of the gestational sac occupied by the collection is more useful than a calculated volume, which can be unreliable because these collections are irregularly shaped.
What causes a subchorionic hematoma?
The exact cause is often not known. A subchorionic hematoma reflects bleeding where the chorionic membranes have separated from the uterine wall. Finding one does not mean that routine activity or something the pregnant person did caused it.
Is bleeding always present with a subchorionic hematoma?
No. Some subchorionic hematomas cause spotting or bleeding, while others are found incidentally on ultrasound. In one assisted-conception cohort, symptoms mattered more than an asymptomatic finding, but that population cannot define the outlook for every pregnancy.
Does a subchorionic hematoma mean miscarriage will happen?
No. Observational studies report different associations depending on gestational age, size definition, symptoms, and conception setting. Some cohorts found higher loss frequencies, while another matched assisted-conception cohort found no significant difference. Ultrasound findings and clinical symptoms together guide counseling.
Should I stay in bed with a subchorionic hematoma?
Routine bed rest is not supported by adequate evidence for subchorionic hematoma. Follow the activity plan given for your specific symptoms and scan findings, but do not start strict bed rest on your own because prolonged inactivity also has downsides.
When is another ultrasound usually done for a subchorionic hematoma?
There is no universal interval. Stable cases commonly receive a follow-up ultrasound, but timing depends on bleeding, gestational age, collection size and location, and what the first scan showed. The obstetric team should give the date rather than the patient choosing an interval.
When does bleeding with a subchorionic hematoma need urgent care?
Contact the obstetric team for any pregnancy bleeding. Seek urgent assessment for heavy bleeding, severe or one-sided pain, shoulder pain, weakness, dizziness, fainting, contractions, fluid leakage, or reduced fetal movement.
