Placental abruption

1st trimester
Placental abruption is primarily a condition of the second half of pregnancy. Bleeding or significant pain earlier in pregnancy still needs assessment because several other urgent conditions can look similar. Do not use the usual timing of abruption to dismiss severe symptoms in the first trimester.
Weeks 14–27
2nd trimester
Abruption is described mainly in the second half of pregnancy. Vaginal bleeding, persistent abdominal or back pain, a tender or tight uterus, repeated contractions, reduced movement after movement has become established, or symptoms after a fall or abdominal blow need urgent assessment, including earlier in the second trimester.
Weeks 28–birth
3rd trimester
Most abruptions occur in the third trimester. Heavy or concealed bleeding can cause maternal instability, and placental separation can reduce fetal oxygen. Go directly to labor and delivery or emergency care for bleeding, sudden persistent pain, a rigid or tender uterus, frequent contractions, reduced fetal movement, dizziness, or faintness.
What does the evidence show for placental abruption?
- ACOG identifies vaginal bleeding with abdominal or back pain as the most common placental abruption pattern. Placental separation can reduce fetal oxygen and cause substantial maternal blood loss, so bleeding in later pregnancy warrants prompt obstetric contact rather than home observation. 1
- MedlinePlus explains that blood may remain trapped behind the placenta, so an abruption can cause cramping, uterine tenderness, contractions, or altered fetal movement without visible bleeding. It also lists a fall, motor vehicle crash, or blow to the abdomen among relevant trauma exposures. 2
- Placental abruption is a clinical emergency assessed with maternal vital signs, bleeding and uterine findings, fetal heart monitoring when gestational age permits, and selected laboratory and imaging studies. When should I call my provider about placental abruption?
Go to labor and delivery or emergency care now for vaginal bleeding with sudden or persistent abdominal or back pain, a tender or rigid uterus, frequent contractions, reduced fetal movement, weakness, dizziness, fainting, pale or clammy skin, or symptoms after significant abdominal trauma. Call emergency services for heavy bleeding, collapse, severe weakness, or inability to travel safely. Absence of visible blood does not make these symptoms safe to watch at home. Can placental abruption happen without vaginal bleeding?
Yes. Blood can remain concealed behind the placenta. Sudden or persistent abdominal or back pain, a tender or unusually firm uterus, frequent contractions, reduced fetal movement, dizziness, or faintness can therefore matter even when no blood is visible.
What do placental abruption symptoms feel like?
The pattern varies. Concerning features include vaginal bleeding, sudden constant abdominal or back pain, uterine tenderness, a tight or rigid-feeling uterus, and contractions that come close together. Some people mainly notice reduced fetal movement or signs of blood loss such as weakness or lightheadedness.
Can placental abruption occur at 16, 18, or 19 weeks?
The cited guidance describes abruption mainly in the second half of pregnancy. Bleeding or significant persistent pain at 16 to 19 weeks still requires urgent assessment. Gestational age and symptoms alone cannot establish which placental or other serious condition is present.
What should I do after a fall or blow to the abdomen?
Contact the obstetric team urgently after significant abdominal trauma, especially with bleeding, pain, contractions, fluid leakage, dizziness, or reduced fetal movement. Abruption can follow trauma, and concealed bleeding means a normal-looking exterior is not a home exclusion test.
Can an ultrasound rule out placental abruption?
No. Ultrasound may show an abruption, but it misses many clinically suspected cases. In one retrospective study, sensitivity was only 24% among patients who delivered within 14 days of scanning. Clinicians combine symptoms, examination, maternal status, fetal monitoring, laboratory results, and imaging.
How is suspected placental abruption assessed in the hospital?
The team checks blood pressure, pulse, bleeding, pain, uterine tenderness and contractions, and fetal heart rate when appropriate. Blood tests may assess anemia and clotting, and ultrasound may look for placental findings or other causes. Monitoring and birth decisions depend on maternal stability, fetal status, gestational age, and severity.
How is placental abruption different from placenta previa?
Both can cause later-pregnancy bleeding. Abruption more often includes pain, uterine tenderness, or frequent contractions, while placenta previa classically causes painless bleeding. Those patterns overlap, so symptoms cannot establish the cause at home and an examination plan should be directed by the obstetric team.
Does placental abruption always mean an immediate cesarean birth?
No single birth route applies to every abruption. Urgent birth may be needed for severe bleeding, maternal instability, abnormal clotting, or fetal compromise. In a stable, limited case remote from term, hospital observation may be considered. Route and timing depend on the maternal and fetal condition and labor progress.
Can fetal movement change with placental abruption?
Yes. Movement may decrease or otherwise change if placental separation affects fetal oxygen. A clear reduction from the baby's usual movement pattern requires immediate obstetric contact, whether or not bleeding or pain is present.
