Circumvallate placenta

Weeks 1–13

1st trimester

Circumvallate placenta is rarely a firm first-trimester conclusion. If an early scan mentions an unusual placental edge, ask whether the finding is provisional and when placental anatomy will be reassessed. Bleeding or severe pain still needs prompt assessment rather than waiting for the anatomy scan.

Weeks 14–27

2nd trimester

Most prenatal impressions arise around the anatomy scan. Ask whether the placenta otherwise looks normal, whether growth and amniotic fluid are appropriate, and whether another placental or cord finding is present. An isolated ultrasound impression is not the same as confirmation by examination of the delivered placenta.

Weeks 28–birth

3rd trimester

Follow the ultrasound schedule chosen for the complete pregnancy picture. New bleeding, contractions, fluid leakage, or reduced fetal movement needs urgent obstetric assessment, regardless of whether a circumvallate edge was suspected. After delivery, placental examination may confirm or refute the prenatal impression.

What does the evidence show for circumvallate placenta?

• Circumvallate placenta is an extrachorial placental form in which the chorionic plate is smaller than the basal plate and the membranes fold back at the margin. It is difficult to identify reliably during pregnancy and is often recognized by examining the placenta after delivery. 1
• A retrospective study of 179 singleton pregnancies with a prenatal ultrasound impression of circumvallate placenta and 716 matched controls found no statistically significant differences in antenatal bleeding, preterm birth, placental abruption, emergent cesarean birth, small-for-gestational-age birth, or stillbirth. 2
• In a blinded sonography study of 62 pregnancies with pathology as the reference, reader performance for circumvallate placenta was poor: most pathology-confirmed cases were missed, and some normal placentas were labeled probably or definitely circumvallate. This supports separating a prenatal impression from pathology confirmation. 3
• An older tertiary-center retrospective study of 92 pathology-confirmed circumvallate placentas reported higher frequencies of preterm birth, placental abruption, small-for-gestational-age birth, emergency cesarean birth, neonatal death, and neonatal intensive care than in 9,057 controls. Its selected pathology-confirmed population is not equivalent to an isolated prenatal ultrasound impression. 4
• A meta-analysis of abnormal placental shapes found circumvallate placenta associated with emergency cesarean birth, preterm birth, and placental abruption, but the authors considered the evidence insufficient for robust conclusions. Included studies were largely retrospective and used varying definitions, often based on postpartum pathology. 5

When should I call my provider about circumvallate placenta?

Seek urgent obstetric assessment for vaginal bleeding, fluid leakage, regular painful contractions, severe or persistent abdominal pain, fainting, or reduced fetal movement. These signs can accompany placental abruption, membrane rupture, preterm labor, or other conditions. A prenatal circumvallate placenta impression is neither proof of the cause nor a reason to delay assessment. When in doubt, call your obstetric team the same day. Early input almost always beats waiting, and the on-call line exists exactly for this.
