1st trimester

Pulmonary embolism can occur in any trimester. Sudden breathlessness, pleuritic chest pain, coughing blood, or fainting needs emergency assessment even early in pregnancy. Pregnancy-adapted pathways may avoid chest imaging in selected patients, but only after structured clinical testing.

Weeks 14–27

2nd trimester

New one-sided leg swelling, pain, warmth, or redness can signal deep vein thrombosis, the usual source of pulmonary emboli. Compression ultrasound can identify a leg clot and may establish the need for care without chest imaging in some cases.

Weeks 28–birth

3rd trimester

Clot risk rises around birth and remains elevated postpartum. Do not dismiss sudden chest symptoms as the mechanical breathlessness of late pregnancy. Imaging choice depends on the clinical pathway, chest findings, local expertise, and test availability.

What does the evidence show for pulmonary embolism?

  • Pregnancy raises venous blood clot risk about fivefold, and the elevated risk continues through three months after birth. CDC lists sudden difficulty breathing, chest pain worsened by a deep breath or cough, coughing blood, and a fast or irregular heartbeat as pulmonary embolism warning signs requiring immediate care. 1
  • Pulmonary embolism is a sudden blockage in a lung artery and can be life-threatening. Shortness of breath, rapid breathing, pleuritic chest pain, rapid heartbeat, coughing blood, low blood pressure, lightheadedness, or fainting are recognized symptoms, but no symptom alone confirms or excludes it. 2
  • In a prospective study of 498 pregnant patients with suspected pulmonary embolism, a pregnancy-adapted YEARS pathway used three clinical criteria, D-dimer thresholds, and leg ultrasound when DVT signs were present. Evaluation combines symptoms, vital signs, examination, clinical probability, blood testing when appropriate, leg compression ultrasound if DVT signs are present, and chest imaging when the pathway cannot safely exclude a lung clot. No home check can substitute for this process.

Can a D-dimer be used during pregnancy?

A D-dimer can be useful inside a validated pregnancy-adapted pathway that combines it with specific clinical criteria. It should not be interpreted as a stand-alone yes-or-no test because levels change in pregnancy and the allowed threshold depends on the pathway findings.

Can leg ultrasound reduce the need for a chest scan?

Yes, when symptoms suggest DVT. The pregnancy-adapted YEARS study used compression ultrasound for patients with DVT signs, and a confirmed leg clot established the need for anticoagulant care without chest CT. A negative leg ultrasound does not exclude a lung clot.

Are CT pulmonary angiography and ventilation-perfusion scans used in pregnancy?

Yes. Both CT pulmonary angiography and ventilation-perfusion scanning are used when chest imaging is needed. A systematic review found comparable very low false-negative rates after a negative result, but study differences and bias made it impossible to declare one test universally superior.

Should fear of radiation delay a needed pulmonary embolism scan?

No. An unrecognized pulmonary embolism can be life-threatening, and both commonly used chest-imaging pathways have been studied in pregnancy. The clinical team selects the most appropriate available test and follows up any nondiagnostic result rather than leaving a serious suspicion unresolved.

How long does pregnancy-related pulmonary embolism risk remain elevated?

The increased clot risk continues throughout pregnancy, around birth, and for three months postpartum according to CDC. New chest or one-sided leg symptoms remain important after delivery, including after an otherwise uncomplicated vaginal or cesarean birth.
