Weeks 1–13

1st trimester

Arrange early joint hematology and obstetric care and review genotype, organ function, transfusion history, antibodies, medicines, and genetic counseling. Obtain a written urgent-pain plan and a pregnancy-appropriate folate and iron plan.

Weeks 14–27

2nd trimester

Keep scheduled blood, urine, and fetal assessments and review hydration, infection concerns, pain episodes, and any hospital visits. Disease severity and pregnancy complications can change the monitoring or transfusion plan.

Weeks 28–birth

3rd trimester

Agree on birth setting, analgesia, blood availability, fluid monitoring, and postpartum follow-up. Growth surveillance and antenatal testing follow the specialist plan; risks do not end at delivery.

What does the evidence show for sickle cell disease?

  • SMFM recommends multidisciplinary care beginning early in pregnancy, including maternal-fetal medicine and hematology. It also recommends fetal growth surveillance and individualized antenatal testing, with earlier or different monitoring when maternal or fetal complications occur. 1
  • A systematic review of 16 studies associated sickle cell disease with higher rates of growth restriction, preterm-related and other perinatal complications, and maternal complications. Observational populations and health-care settings varied, so pooled risks are not an individual prognosis. 2
  • WHO’s 2025 guidance emphasizes individualized pain care, nutrition, medication review, and monitoring. It advises against routine iron supplementation without evidence of deficiency and allows selected hydroxyurea decisions through a specialist risk-benefit discussion rather than an automatic blanket rule. 3
  • A meta-analysis of 12 studies found associations favoring prophylactic transfusion for several outcomes, but studies had substantial methodological limitations. This supports a specialist discussion, not routine transfusion for every pregnancy with sickle cell disease. 4
  • SMFM recommends a folic-acid plan and prenatal vitamins without iron unless iron deficiency is confirmed. It recommends shared decision-making about hydroxyurea that accounts for timing and disease severity; stopping a disease-control medicine without advice can also carry risk. 1

When should I call my provider about sickle cell disease?

Seek emergency care for chest pain, shortness of breath, new weakness or speech difficulty, seizure, fainting, or severe uncontrolled pain. Fever in sickle cell disease warrants urgent assessment. Contact maternity care urgently for reduced fetal movement, bleeding, contractions, or concerning headache or blood pressure. Use the agreed crisis plan and tell emergency staff that you are pregnant and have sickle cell disease. Trust your read on your own body. Frequently asked questions

Is sickle cell trait the same as sickle cell disease in pregnancy?

No. Trait and disease have different clinical implications. Confirm the specific genotype with the clinician, and ask about partner testing and genetic counseling. A page about sickle cell disease should not be used to assign its risks to every carrier.

Which clinicians should coordinate my pregnancy care?

Maternal-fetal medicine and a hematologist familiar with sickle cell disease should lead a coordinated plan. Pain specialists, genetics, transfusion services, and psychosocial support may also be involved. The plan should identify one contact route for urgent symptoms.

Should I take an iron-containing prenatal vitamin?

Not automatically. Chronic anemia in sickle cell disease does not itself prove iron deficiency, and prior transfusions can change iron status. SMFM and WHO recommend iron only when deficiency is established; obtain an individualized prenatal and folate plan.

Must hydroxyurea always be stopped throughout pregnancy?

Current guidance calls for individualized specialist decisions. WHO allows selected continuation or restarting after the first trimester in previously controlled patients, while weighing severity, alternatives, timing, and patient preferences. Do not start, stop, or restart it on your own.

Will I need regular preventive transfusions?

Possibly, but not everyone does. Prior severe crises, previous response, current complications, transfusion antibodies, and transfusion risks matter. Evidence is limited, so hematology and maternal-fetal medicine should explain the expected benefit and uncertainty for your history.

What should a pregnancy pain plan include?

It should identify medicines and timing appropriate to the pregnancy stage, steps for prompt assessment, and when hospital care is needed. Severe pain should receive timely care; do not assume that all chest or abdominal pain is an uncomplicated familiar crisis.

Does sickle cell disease always mean a cesarean birth?

No. Birth route and timing depend on obstetric circumstances, maternal status, fetal findings, and the specialist plan. Delivery planning should also cover pain control, careful fluid management, blood availability, and postpartum monitoring.

What follow-up is needed after birth?

Continue hematology and obstetric follow-up for pain, infection, clotting concerns, anemia, and medication decisions. Review breastfeeding compatibility medicine by medicine, arrange support for recovery, and discuss future pregnancy planning when you are ready.
