Low PAPP-A in pregnancy

Weeks 1–13
1st trimester
PAPP-A is interpreted as part of a first-trimester screening calculation with gestational age and other markers. A low value can raise screening risk for trisomies, but a screening result is not confirmation and should be discussed with the prenatal testing team.
Weeks 14–27
2nd trimester
The anatomy scan and any clinician-selected growth assessment add information that one PAPP-A value cannot provide. Low PAPP-A has statistical associations with placental outcomes, yet large cohort data show poor positive predictive value when it is used alone.
Weeks 28–birth
3rd trimester
Growth follow-up and antenatal testing depend on the value, ultrasound findings, medical history, and local plan. ACOG states that weekly surveillance may be considered from 36 weeks when first-trimester PAPP-A is at or below the fifth percentile even if growth appears normal.
What does the evidence show for low PAPP-A?
- PAPP-A is one component of first-trimester serum screening, and lower levels can occur with trisomies 21, 18, and 13; the combined screen estimates risk and does not confirm a chromosome condition. 1
- A nested case-control study comparing 248 pregnancies with adverse outcomes and 244 gestational-age-matched controls found an adjusted odds ratio of 2.1 for a composite adverse outcome with PAPP-A at or below 0.4 MoM, but sensitivity was only 38.7%. 2
- In a prospective low-risk cohort of 4,739 first pregnancies, PAPP-A below 0.4 MoM had poor positive predictive values for preeclampsia, a newborn below the third growth percentile, and fetal death, so isolated low PAPP-A could not predict an individual outcome. 3
- ACOG states that serum screening results can inform decisions about serial growth assessment and that weekly antenatal surveillance may be considered from 36 weeks when first-trimester PAPP-A is at or below the fifth percentile, even with normal growth. 4
- Flashes, blind spots, persistent blurry vision, severe unrelenting headache, severe belly pain, trouble breathing, or a clear reduction in fetal movement are urgent pregnancy warning signs regardless of a PAPP-A result. 5
When should I call my provider about low PAPP-A?
Low PAPP-A itself has no emergency symptom. Seek immediate pregnancy care for a severe headache that does not ease, flashes or blind spots, persistent blurry vision, severe abdominal pain, trouble breathing, vaginal bleeding or fluid leakage, or a clear reduction in fetal movement. These warning signs need assessment on their own and should not wait for a scheduled growth scan or surveillance visit. When in doubt, call your obstetric team the same day. Early input almost always beats waiting, and the on-call line exists exactly for this.

What does a low PAPP-A result mean?
Low PAPP-A means this placental protein measured below the laboratory's reference for gestational age, often reported in multiples of the median. It is a marker used in a risk calculation, not a condition you can feel and not a result that predicts an adverse outcome by itself.
Does low PAPP-A mean the baby has Down syndrome?
No. Low PAPP-A can contribute to a higher first-trimester screening estimate for trisomy 21, but it does not confirm Down syndrome. The complete screen includes other information, and follow-up choices may include cell-free DNA screening, genetic counseling, or a diagnostic procedure based on the full result and your preferences.
Can low PAPP-A predict fetal growth restriction?
Low PAPP-A is associated with a higher group-level chance of placental outcomes, including a very small newborn, but prediction for one pregnancy is poor. In a 4,739-participant low-risk cohort, PAPP-A below 0.4 MoM had a positive predictive value of 4.4% for birthweight below the third percentile.
Does low PAPP-A cause symptoms during pregnancy?
Low PAPP-A itself does not cause symptoms. It is a laboratory marker. Keep routine prenatal visits and follow the monitoring plan your clinician sets. New severe headache, visual changes, severe abdominal pain, trouble breathing, or reduced fetal movement needs prompt attention because symptoms matter independently of the screening marker.
Will I need extra growth ultrasounds for low PAPP-A?
Some clinicians use the complete serum-screen result to decide whether serial growth ultrasounds are appropriate. The schedule is individualized because an isolated low PAPP-A value has limited predictive value. Ultrasound findings, the exact percentile, blood pressure, medical history, and other placental risk factors all affect the plan.
Will low PAPP-A require nonstress tests late in pregnancy?
ACOG says weekly antenatal fetal surveillance may be considered from 36 weeks when first-trimester PAPP-A is at or below the fifth percentile, even if growth is normal. The wording is permissive, not a universal mandate, so your obstetric team should tailor the timing to the whole clinical picture.
Should I start aspirin because my PAPP-A is low?
Do not start aspirin from a PAPP-A result alone. In the low-risk GOS cohort, investigators concluded that isolated PAPP-A below 0.4 MoM was not an indication for low-dose aspirin. Your clinician may recommend aspirin for separate preeclampsia risk factors, so the decision requires a full risk review.
Can PAPP-A be repeated later to show that the risk is gone?
PAPP-A is designed for a gestational-age-specific first-trimester screening calculation, not as a serial reassurance test. Later assessment uses other information, such as anatomy, fetal growth, blood pressure, symptoms, and clinician-selected fetal surveillance. A repeat number outside the validated screening window would not answer the same question.
