NIPT low fetal fraction no result
By trimester
Weeks 1–13
1st trimester
Before or around 10 weeks, the placental DNA fraction may be too low for an assay to report. Promptly review exact gestational dating, the laboratory report, ultrasound findings, and whether a redraw would still leave time for desired testing choices.
Weeks 14–27
2nd trimester
A no-result later in pregnancy deserves timely counseling because waiting for another sample can narrow testing choices. Detailed ultrasound and discussion of diagnostic testing, usually amniocentesis at this stage, may be more informative than repeated screening in some situations.
Weeks 28–birth
3rd trimester
A prior low-fetal-fraction result is not a third-trimester risk score. Follow the individualized plan based on subsequent screening, ultrasound, fetal growth, blood pressure, and any diagnostic results rather than assuming the no-result predicts an outcome.
What does the evidence show for NIPT low fetal fraction no result?
- MedlinePlus explains that NIPT analyzes placental cell-free DNA and remains a screening test. A fetal fraction below the assay requirement can yield no result or a false negative; early sampling, maternal obesity, sampling error, or fetal abnormality can contribute. 1
- ACOG's 2026 advisory says a nonreportable cell-free DNA result is associated with increased aneuploidy risk and should prompt genetic counseling, comprehensive ultrasound, and an offer of diagnostic testing. A redraw decision should account for ultrasound findings and gestational age. 2
- A retrospective commercial-laboratory study of 4,018 initial no-result cases reported an informative second sample in 70.6% overall and 62.9% of insufficient-fetal-fraction cases. Redraw success depended strongly on maternal weight and the first sample's fetal fraction; the study did not compare fetal outcomes. 3
- Updated SMFM guidance describes gestational age, maternal weight, technical factors, assisted reproduction, some maternal conditions or medicines, placental mosaicism, and multifetal pregnancy as contributors to low fetal fraction or nonreportable results. It also emphasizes that screening does not replace diagnostic testing. 4
- A 2024 meta-analysis of eight observational studies with 72,507 participants associated low fetal fraction with hypertensive disorders, gestational diabetes, and small-for-gestational-age or growth-restricted infants, while the preterm-birth association was not significant. When should I call my provider about NIPT low fetal fraction no result?
A low-fetal-fraction report is not itself an emergency, but contact the ordering clinician promptly because follow-up choices are gestational-age sensitive. Seek urgent pregnancy assessment for heavy bleeding, severe abdominal pain, fluid leakage, or other acute warning symptoms rather than attributing them to the screening result. When in doubt, call your obstetric team the same day. Early input almost always beats waiting, and the on-call line exists exactly for this.
Frequently asked questions
Does low fetal fraction mean the baby has a chromosome condition?
No. It means the sample could not support a reliable report. Some chromosome conditions are more common among low-fraction results, so the result is not low risk and deserves counseling and follow-up.
Can testing at 10 weeks cause a low fetal fraction?
Yes. Placental cell-free DNA generally rises with gestational age, and an early draw can contribute. A result near 10 weeks still needs individualized review because gestational age is only one possible factor.
Does maternal BMI affect fetal fraction?
Higher maternal weight is associated with lower proportional fetal fraction and lower redraw success. This is a laboratory limitation, not a judgment about the patient, and it does not by itself identify a fetal condition.
How often does a repeat NIPT produce a result?
One commercial-laboratory study reported a result after redraw in 70.6% of all initial no-result cases and 62.9% when insufficient fetal fraction caused the failure. Rates vary by assay, weight, initial fraction, and population.
Should I repeat NIPT or choose a diagnostic procedure?
That choice depends on gestational age, ultrasound findings, baseline risk, assay reason, and how much certainty you want. Current ACOG guidance calls for counseling, comprehensive ultrasound, and an offer of diagnostic testing after a nonreportable result.
Is a second low fetal fraction result reassuring?
No. Two failures still do not provide a negative screen. Repeated nonreportable results should prompt renewed counseling about ultrasound and diagnostic options rather than being interpreted as normal.
How does a twin pregnancy affect fetal fraction?
In twins, the total placental DNA signal and each fetus's contribution can differ, especially with dizygotic twins. Test performance and the meaning of a no-result therefore depend on the assay and chorionicity.
Does low fetal fraction predict preeclampsia or poor growth?
Observational studies report associations with hypertensive disorders and impaired growth, but low fetal fraction is not a validated stand-alone forecast for an individual pregnancy. Follow-up should be based on the full obstetric picture.
