Humira vs Enbrel vs Cimzia During Pregnancy

Assigned clinical reviewerMatt Stahl· PharmDDraft · pending clinical reviewUpdated

How do the options compare?

The lens is a clinician's: guideline endorsement, pregnancy evidence, dosing, onset, and tolerability. Cost and formulation notes appear where relevant.

OptionStudied pregnancy doseGuideline supportNotes
Adalimumab (Humira)Use the prescribed regimenEvidence summary with specialist reviewMotherToBaby summarizes several hundred exposed pregnancies and notes more placental passage later in pregnancy.
Etanercept (Enbrel)Use the prescribed regimenEvidence summary with specialist reviewAvailable studies are generally reassuring, but disease activity and the reason for use remain central to planning.
Certolizumab (Cimzia)Use the prescribed regimenEvidence summary with specialist reviewLittle to no placental transfer in measured samples. Different study populations and stopping schedules limit direct comparisons with etanercept.
  • MotherToBaby reports that studies of nearly 700 adalimumab-exposed pregnancies did not show a higher chance of birth defects, while disease-related factors can complicate interpretation of other findings. 1
  • MotherToBaby reports that most etanercept studies do not show a higher chance of birth defects, while some studies cannot separate medication effects from the underlying disease. 2
  • MotherToBaby describes little to no placental transfer of certolizumab in measured cord and infant blood samples and reports no higher birth-defect rate in published exposure data. 3
  • A study of 31 pregnancies with inflammatory bowel disease compared infliximab, adalimumab, and certolizumab. Certolizumab had much lower placental transfer than the two antibodies. Etanercept was not included, so this was not a direct comparison of all three options on this page. 4
  • A cord-blood study included certolizumab, etanercept, adalimumab, and infliximab after different medication-stopping schedules. Etanercept was undetectable in its 30 samples; certolizumab was detectable in 5.9% of 68 samples. Different timing limits direct drug rankings. 5

Which option makes sense?

The comparison above is intentionally evidence-first, not brand-first. Please consult your healthcare provider to translate it into a plan that fits your pregnancy specifics.

Frequently asked questions

How do placental-transfer studies compare these TNF inhibitors?

Certolizumab shows little to no transfer in pregnancy studies. An inflammatory-bowel-disease study compared it with adalimumab and infliximab, not etanercept. Another study included etanercept but used different stopping schedules, so its cord-blood levels cannot establish a simple ranking for continuous use.

Is certolizumab always the best TNF inhibitor in pregnancy?

No. Lower placental transfer is one consideration, but disease control, prior response, condition, and the risks of changing a working medicine also matter.

Can adalimumab be used during pregnancy?

MotherToBaby summarizes studies of adalimumab exposure that did not show a higher birth-defect rate in several hundred pregnancies. Decisions still need specialist review.

Can etanercept be used during pregnancy?

Most etanercept studies summarized by MotherToBaby do not show a higher birth-defect rate. Individual planning should consider the inflammatory condition and disease activity.

Should I switch from adalimumab to certolizumab after a positive pregnancy test?

Do not switch a biologic medication on your own. A rheumatology, gastroenterology, dermatology, or maternal-fetal specialist can weigh disease control against exposure considerations.

Do TNF inhibitors affect newborn vaccines?

Biologic exposure late in pregnancy can affect newborn vaccine planning. The prescribing specialist and pediatric clinician should know which medicine was used and when.

Can I breastfeed on a TNF inhibitor?

Breastfeeding questions differ by medicine and dose. MotherToBaby has medicine-specific information, and the prescribing and pediatric teams can advise on the individual plan.

Does active autoimmune disease matter in pregnancy decisions?

Yes. Active autoimmune or inflammatory disease can itself affect pregnancy outcomes, so a medication decision should compare the risks of exposure with the risks of losing disease control.
