Certolizumab (Cimzia) During Pregnancy: Placental Transfer
What does the evidence say about certolizumab pegol during pregnancy?
- MotherToBaby reports more than 2,000 certolizumab-exposed pregnancies without a higher rate of birth differences in the available data, while noting that active autoimmune disease itself can affect pregnancy outcomes. 1
- In CRIB, 13 of 14 evaluable infants had no quantifiable certolizumab in plasma at birth and one had 0.042 micrograms per milliliter; no infant had a quantifiable level at weeks four or eight. 2
- MotherToBaby reports that studies have not found a higher miscarriage rate with certolizumab. This clinical-outcome evidence is separate from the small CRIB study of drug transfer and does not rule out every individual risk. 1

Is certolizumab pegol safe in each trimester?
- First trimester. More than 2,000 reported pregnancy exposures have not shown a higher overall rate of birth differences, but these observational data cannot rule out uncommon outcomes or fully separate medicine effects from the underlying inflammatory condition.
- Second trimester. Keeping inflammatory disease controlled remains part of the risk-benefit decision. Certolizumab lacks the Fc region used by intact IgG antibodies for active placental transfer, but that structural distinction does not replace clinical monitoring.
- Third trimester. CRIB directly measured maternal, cord, and infant concentrations after late-pregnancy dosing and found minimal transfer. Its 14 evaluable infant samples were too few to establish uncommon birth or long-term developmental outcomes.

Does Cimzia cross the placenta?
CRIB found minimal third-trimester placental transfer. Thirteen of 14 evaluable infants had no quantifiable certolizumab in plasma at birth, while one had a very low level. The small study did not establish every fetal or long-term outcome.
Why is certolizumab different from other TNF inhibitors?
Certolizumab lacks the antibody Fc region used in active Fc-receptor transport across the placenta. That structural feature explains its distinctive transfer profile, but it does not make certolizumab automatically right for every inflammatory condition or pregnancy.
Does Cimzia raise the chance of miscarriage?
Studies summarized by MotherToBaby have not found a higher miscarriage rate with certolizumab. That finding does not mean every pregnancy outcome can be predicted. Your clinician should also consider the underlying inflammatory condition, disease activity, and any other medicines.
Does the Cimzia dose need to change during pregnancy?
Do not adjust Cimzia on your own. The CHERISH analysis of maternal drug concentrations supported established regimens during pregnancy, but it did not test fetal outcomes. The prescribing specialist should review disease control, symptoms, and the individual dosing plan.
Can Cimzia be continued throughout pregnancy?
A specialist may continue Cimzia when maintaining control of inflammatory disease warrants it. The plan should account for disease activity, prior response, infection history, other medicines, dose timing, and the limits of observational outcome data.
Should Cimzia be stopped after a positive pregnancy test?
Do not stop Cimzia solely because of a positive test. Contact the prescribing specialist and prenatal clinician promptly so they can compare medicine exposure with the consequences of an inflammatory flare and agree on the next dose.
Can Cimzia be used while breastfeeding?
CRADLE found minimal or no certolizumab in mature breast milk and estimated a median relative infant dose of 0.15%. The study involved 17 mothers and did not establish long-term outcomes, so include the infant clinician in the plan.
Does minimal placental transfer settle newborn vaccine timing?
No. Tell the pediatric clinician about Cimzia dose dates and any other biologic exposure. Vaccine decisions consider the specific medicine, timing, infant health, and local guidance; a small transfer study cannot replace an individualized newborn plan.
