Scabies during pregnancy
By trimester
Weeks 1–13
1st trimester
Confirm the cause before applying a pesticide medicine to a widespread rash. Pregnancy exposure data are strongest for topical permethrin, while CDC says the safety of oral ivermectin in pregnancy has not been established.
Weeks 14–27
2nd trimester
Apply the clinician-selected topical product exactly as directed and coordinate same-time care for household members and close skin contacts. Clean recently used fabrics on the same cycle so reinfestation is less likely.
Weeks 28–birth
3rd trimester
Persistent itching alone does not necessarily mean active mites because itch can last several weeks. New burrows, new pimple-like lesions, crusting, or itch beyond two to four weeks merits reassessment before another application.
What does the evidence show for scabies?
- MotherToBaby describes scabies as an intensely itchy mite infestation spread mainly through close contact. Scabies itself is not passed directly to the fetus, and published human pregnancy information has not shown a specific pattern of adverse pregnancy outcomes. 1
- CDC lists permethrin 5% cream as an established scabies medicine and notes that oral ivermectin has similar efficacy but unestablished pregnancy safety. Lindane should not be used by pregnant or breastfeeding people. 2
- CDC advises washing recently used bedding and clothing in hot water and drying hot, or sealing items that cannot be washed for several days. Close contacts should be notified, and close skin contact should pause until the care course is complete. 3
- An international clinical guideline reports that skin scraping microscopy has high specificity but limited sensitivity, so a negative scraping does not exclude scabies. Clinical history, lesion distribution, contact history, dermoscopy, or microscopy may all contribute to confirmation. 4
- Itching can continue for several weeks after successful mite clearance. CDC advises reassessment when itching lasts beyond two to four weeks or when new burrows or new pimple-like lesions appear, rather than assuming every residual itch is active infestation. 3
When should I call my provider about scabies?
Seek prompt medical care for fever, rapidly spreading redness, warmth, swelling, pus, severe pain, or red streaking that may signal a bacterial skin infection. Extensive crusting, involvement of an immunocompromised household member, eye exposure to a topical product, breathing trouble, facial swelling, or another severe medicine reaction needs urgent assessment. Whatever the pattern, any symptom that feels beyond your usual range is a reason to call: obstetric teams expect these questions and would rather hear early than late.
Frequently asked questions
What does scabies look and feel like during pregnancy?
Scabies often causes intense itching that is worse at night, small pimple-like lesions, and thin burrows. Common sites include finger webs, wrists, waist, buttocks, nipples, and genital skin. Pregnancy rashes can look similar, so appearance alone is not enough.
How is scabies confirmed during pregnancy?
A clinician considers the itch pattern, lesion distribution, burrows, and close-contact history. Dermoscopy or a skin scraping can identify mites, eggs, or fecal material. A negative scraping cannot fully exclude scabies because the test misses some genuine infestations.
Which scabies medicine is best established in pregnancy?
Topical permethrin 5% cream has the strongest pregnancy experience and is generally the preferred option. CDC directions describe application from neck downward for 8 to 14 hours, sometimes repeated about a week later. A clinician should confirm the exact plan and body areas.
Can oral ivermectin be used for scabies during pregnancy?
CDC states that oral ivermectin's safety in pregnancy has not been established. MotherToBaby recommends better-studied options first. Do not take veterinary ivermectin or use someone else's prescription; a specialist should weigh alternatives in an unusually severe or difficult case.
Does everyone in the household need scabies care?
Household members and close skin contacts should be evaluated and usually managed at the same time, even if symptoms have not appeared yet. Coordinating the timing reduces the chance that an untreated contact passes mites back after the pregnant person finishes care.
How should bedding and clothes be handled for scabies?
Wash recently used clothing, towels, and bedding in hot water and dry them on a hot cycle. Seal items that cannot be washed in a closed bag for several days to a week. Routine insecticide sprays or fumigation are not needed.
How long can itching last after scabies care?
Itching can continue for several weeks even after the mites are gone because the allergic skin reaction takes time to settle. Contact the clinician if itch persists beyond two to four weeks or if new burrows or new pimple-like lesions develop.
Can scabies harm the fetus?
Scabies mites do not pass directly to the fetus. Human pregnancy information has not linked scabies itself with a specific birth-defect pattern, but severe scratching can damage skin and lead to bacterial infection that needs medical attention.
When can close skin contact resume after scabies?
Avoid close skin-to-skin and sexual contact until the prescribed course is complete and contacts have been managed. Follow the clinician's timing for the selected product, since persistent post-scabies itch does not necessarily mean the person remains contagious.
