Is Permethrin Safe During Pregnancy for Lice or Scabies?

Assigned clinical reviewerMatt Stahl· PharmDDraft · pending clinical reviewUpdated

What does the evidence say about permethrin during pregnancy?

  1. MotherToBaby states that lice and scabies themselves do not pass to the fetus and that absorption from topical creams, lotions, and shampoos is generally low. Available topical-product studies have not suggested a higher birth-defect risk, but data vary by medicine and are limited. 1
  2. A clinical review estimated about 2% systemic absorption from topical permethrin. It summarized a prospective cohort of 113 pregnant permethrin-shampoo users, including 31 first-trimester exposures, and another 196 pregnancies exposed to 4% permethrin, with no adverse fetal signal reported in either study. 2
  3. CDC identifies 5% permethrin cream as a prescription scabies medicine applied from the neck down and washed off after 8 to 14 hours. It notes that no over-the-counter scabies product is approved and that oral ivermectin pregnancy safety is not established. 3
  4. CDC sexually transmitted infection guidance lists 5% permethrin cream for scabies in pregnancy and says lindane should not be used during pregnancy. It also notes that itching can persist for under two weeks after scabies medicine, so symptoms alone do not always mean the application failed. 4
  5. The current 5% permethrin cream label reports no adequate controlled studies in pregnant women, directs use only when clearly needed, and describes generally mild local effects such as burning, stinging, itching, redness, or rash. The label's animal findings cannot replace human pregnancy data. 5

Is permethrin safe in each trimester?

  • First trimester. First-trimester human data include 31 exposures in one prospective permethrin-shampoo cohort, with no embryotoxicity signal reported. The sample is small, so confirm that live lice or scabies is actually present and use the exact product directions rather than applying extra product for reassurance.
  • Second trimester. Low topical absorption limits systemic exposure, but lice and scabies use different strengths and application patterns. Ask a pharmacist or prenatal clinician to verify which condition is present, which areas need application, and whether household contacts or bedding also need coordinated care.
  • Third trimester. Late-pregnancy use still requires the correct formulation and contact time. Persistent itching after scabies cream does not by itself prove live mites remain, so do not automatically repeat applications without checking for new burrows, live mites, irritation, or another cause.

Frequently asked questions

Can you use permethrin cream while pregnant?

Permethrin cream has low skin absorption and limited reassuring human pregnancy data. For scabies, the usual U.S. product is prescription 5% cream. Confirm the condition and follow the prescribed application area and contact time rather than using a lice product interchangeably.

Is permethrin lice shampoo safe during pregnancy?

Available permethrin-shampoo pregnancy data have not shown an embryotoxicity signal, including a small number of first-trimester exposures. The evidence is not large enough to promise zero risk, so verify live lice and use the labeled amount and repeat schedule.

Does permethrin absorb into the bloodstream through the skin?

Yes, but the reviewed estimate for topical permethrin was about 2% systemic absorption. Absorption may still vary with formulation, skin condition, body area, and application, which is why more product or longer contact is not automatically better.

What should you use instead of permethrin during the first trimester?

There is no evidence-based rule that every first-trimester case needs an alternative to permethrin. Mechanical nit removal can support head-lice care, while suspected scabies needs clinician confirmation. Lindane is not a preferred substitute in pregnancy, and oral ivermectin has less established pregnancy safety.

Are lice permethrin and scabies permethrin the same product?

No. Head-lice products commonly use 1% permethrin on hair and scalp, while U.S. scabies care uses prescription 5% cream over a much larger skin area for a longer contact time. Do not substitute one strength or set of directions for the other.

Does itching after permethrin mean scabies is still active?

Not necessarily. CDC notes that itching can persist for under two weeks after scabies medicine. New burrows, live mites, worsening rash, or symptoms beyond the expected interval are better reasons to seek reassessment than itching alone.

Should household members use permethrin too?

Scabies and lice can spread through close contact, so coordinated household advice may be important, but each person needs age-appropriate directions and confirmation of the condition. Do not automatically apply a pregnant person's prescription cream to other family members.

When should a pregnant person call after using permethrin?

Contact a healthcare professional for severe burning, marked swelling, spreading rash, breathing difficulty, eye exposure, accidental ingestion, or symptoms that do not fit lice or scabies. Mild temporary stinging or itching can occur, but escalating symptoms need reassessment.
