Could sudden pregnancy pain be ovarian torsion?
What does the evidence show for ovarian torsion?
- Twisting can obstruct drainage and eventually damage ovarian tissue. It can involve an enlarged ovary or mass, but it also occurs in an ovary without a known cyst. The possibility is assessed from symptoms, examination and imaging together. 1
- Pregnancy and enlarged ovaries after fertility stimulation are relevant clinical context. Tell the emergency team about recent fertility care and any scan showing an ovarian mass. A risk factor increases concern but cannot establish the cause of pain by itself. 2
- Doppler ultrasound may still show flow when torsion is present because ovarian blood supply and the degree of twisting vary. Persistent clinical concern should not be dismissed solely because a report mentions blood flow. Specialists interpret the overall findings. 1
- Ectopic pregnancy can also produce one-sided pain, bleeding, shoulder pain or faintness, particularly early in pregnancy. Distinguishing these emergencies requires clinical evaluation. Do not choose between them using a symptom checklist or wait for all possible symptoms to appear. 3
- Severe pain that starts suddenly or continues is a reason to seek immediate care during pregnancy. If you feel faint or unstable, use emergency services rather than driving yourself. Mention the pregnancy even when pain seems to be centered around an ovary. 4

Frequently asked questions
Can torsion happen without an ovarian cyst?
Yes. An enlarged ovary or mass is a common association, but torsion can also involve an otherwise normal ovary.
Does visible blood flow on ultrasound rule it out?
No. Doppler findings vary, and maintained flow does not reliably exclude torsion when the symptoms and other findings remain concerning.
Does fertility stimulation matter to the assessment?
Yes. Stimulation can enlarge the ovaries, which is relevant to torsion risk. Give the emergency team the timing of your cycle and procedures.
Can I wait to see whether severe pain settles overnight?
Do not delay immediate assessment for sudden severe or persistent abdominal pain during pregnancy. Temporary improvement does not supply a reliable explanation.
Could surgery be discussed while I am pregnant?
Yes. A suspected serious complication of an ovarian mass can require urgent surgical assessment. The team weighs maternal, ovarian and pregnancy considerations together.
How is this different from an ectopic pregnancy?
Torsion involves a twisted ovary; ectopic pregnancy involves a pregnancy outside the uterus. Symptoms can overlap, and testing is needed to distinguish causes.
Is a cyst measurement enough to predict torsion?
No. Size is part of the context, but the clinical situation and imaging findings matter. A measurement is not an at-home rule for ruling torsion in or out.
What information should I give the emergency team?
Describe when pain began, its location and severity, vomiting, bleeding or faintness, and your pregnancy stage. State clearly if symptoms are worsening.
