Dry heaving during pregnancy

By trimester

Weeks 1–13

1st trimester

Overnight is the longest stretch you go without food, so waking up is prime time for the empty-stomach retch cycle to start. Keep a small protein bite, like a hard-boiled egg or a cheese stick, right at your bedside and eat a few bites before you even sit up. Carry the same habit into the rest of the morning with a bite every hour or two.

Weeks 14–27

2nd trimester

Dry heaving usually fades out between weeks 12 and 16 for most women. If it's still showing up, especially first thing in the morning, keep the pre-rising snack habit going and mention it at your next appointment so your provider can see how it's tracking.

Weeks 28–birth

3rd trimester

New dry heaving that shows up for the first time this late deserves a call to your provider rather than the standard mini-meal fix, since third-trimester causes can be different from early pregnancy nausea. Don't assume it's the same empty-stomach pattern without checking.

What does the evidence show for dry heaving?

  • Retching without throwing up, often called dry heaving, is named as one of the core symptoms of nausea and vomiting of pregnancy, and it typically starts around 4 to 9 weeks and peaks between 7 and 12 weeks. 1
  • Eating a dry cracker or plain toast before you even sit up gives your stomach something to work with before you start moving, which is exactly the moment an empty-stomach retch cycle tends to kick in. 2
  • Building a bite of protein, like a spoonful of yogurt or a few almonds, into a snack every hour or two keeps your stomach from running empty again once you're up and moving. 1
  • Sipping something cold through the morning instead of gulping a full glass at breakfast adds fluid without the stomach fullness that can trigger another retch, and spacing drinks about 20 to 30 minutes from meals follows the same logic. 1
  • When diet and fluid changes alone aren't breaking the cycle and you're losing weight or showing signs of dehydration, ACOG's clinical guidance calls for stepping up to added nutrition support rather than waiting it out at home. 3

When should I call my provider about dry heaving?

Retching that starts for the first time at 10 weeks of pregnancy or later, or that starts interfering with your ability to eat, drink, sleep, or get through your day, is worth bringing to your provider rather than assuming it is routine morning sickness. The same goes for dry heaving that leaves you unable to keep any food or fluids down, since that pattern can cross into hyperemesis gravidarum, which involves real weight loss and dehydration and sometimes needs hospital-based care. If you are ever unsure which side of the line you are on, make the call. Obstetric teams handle these check-ins as routine, not as overreacting.

Frequently asked questions

Is dry heaving the same thing as morning sickness?

Not exactly. Morning sickness is the everyday name for nausea and vomiting of pregnancy, and dry heaving, or retching without bringing anything up, is one specific way that can show up. It's named right alongside nausea and vomiting as a recognized symptom, so having it doesn't mean something extra is wrong. Consult your healthcare provider if it's constant or keeps you from eating.

Why do I dry heave first thing when I wake up?

Your stomach has been empty for hours by the time you wake up, and moving around on an empty stomach is one of the strongest triggers for retching. Keeping a small bite, like crackers or a cheese stick, at your bedside and eating it before you get up gives your stomach something to work with before that trigger hits.

What actually breaks the dry heaving cycle during the day?

Small bites with some protein every 1 to 2 hours, instead of three big meals, keep your stomach from emptying out again. Pair that with sipping cold fluids between meals rather than large amounts with food, and the two habits together tend to calm the cycle better than either one alone.

When does dry heaving mean I need to call my provider?

Reach out the same day if you can't keep any food or fluids down for half a day, you're losing weight, or you feel dizzy or lightheaded when standing. Those can be signs of a more serious condition called hyperemesis gravidarum, and it needs real medical support, not just a diet tweak.
