Double vision during pregnancy

Weeks 1–13

1st trimester

True double vision is uncommon this early. Severe, persistent vomiting is the first-trimester cause most worth knowing about, since it can lead to a thiamine deficiency that affects eye movement and needs urgent replacement rather than being dismissed as ordinary morning sickness.

Weeks 14–27

2nd trimester

Report any new double vision now rather than waiting to see if it passes. Telling a benign eye-alignment issue apart from a nerve or pressure-related cause takes an exam, and covering one eye at home can narrow the possibilities but cannot rule out a serious cause on its own.

Weeks 28–birth

3rd trimester

Preeclampsia becomes a more relevant cause of double vision in the third trimester, often through a nerve that controls eye movement. Double vision alongside headache, high blood pressure, or sudden swelling needs same-day evaluation rather than waiting for a scheduled visit.

What does the evidence show for double vision?

• In a cross-sectional study of 562 women seen in an eye emergency setting, transient visual disturbances with an otherwise normal eye exam occurred in 48% of the 69 pregnant women, compared with 11% of non-pregnant women, a difference that held up after adjusting for other factors. Double vision specifically was uncommon, making up only about 1% of all reported visual complaints in the study. 1
• Sixth cranial nerve palsies, a cause of double vision, are associated with gestational hypertension and preeclampsia and may point to elevated pressure inside the skull or a blood clot in the brain's venous sinuses, both of which need prompt evaluation. 2
• Severe, persistent nausea and vomiting in pregnancy can lead to a thiamine deficiency that causes double vision along with abnormal eye movements, and this needs urgent thiamine replacement rather than being managed as ordinary morning sickness. 2
• A pituitary tumor, pituitary apoplexy, or an expanding aneurysm near the base of the brain can each cause double vision through pressure on the nerves that move the eyes, and any third cranial nerve palsy specifically needs emergency imaging to rule out an aneurysm. 2
• Double vision is first sorted into two types: monocular, which persists even with one eye closed and usually points to a problem within that eye itself, or binocular, which resolves when either eye is covered and points to a problem with eye alignment or the nerves controlling it. 3
• Double vision can also appear with a pituitary adenoma or pituitary apoplexy during pregnancy, alongside symptoms like a sudden severe headache, a drooping eyelid, or an enlarged pupil, a symptom pattern that points away from a benign explanation. 4

Sorting out double vision in pregnancy

When should I call my provider about double vision?

New double vision together with a severe headache, high blood pressure, or sudden swelling needs same-day evaluation for possible preeclampsia. A drooping eyelid, an enlarged or unequal pupil, or double vision that comes on suddenly and severely needs emergency evaluation for a possible aneurysm or pituitary problem. Double vision together with severe, persistent vomiting also needs prompt evaluation for possible thiamine deficiency. When in doubt, call your obstetric team the same day. Early input almost always beats waiting, and the on-call line exists exactly for this.
