Is Prozac safe during pregnancy?
What does the evidence say about Prozac during pregnancy?
- Fluoxetine use is not expected to increase the chance of birth defects, and a small number of studies found no greater chance of miscarriage when it was used in pregnancy. 1

Is Prozac safe in each trimester?
- First trimester. The first-trimester picture is mostly reassuring with one caveat. A Bayesian analysis pooling data from nearly 28,000 mothers found elevated estimates for right ventricular outflow tract obstruction and craniosynostosis, roughly double the background rate, while most other defects showed no association at all.
- Second trimester. Mid-pregnancy fluoxetine use is studied less than early exposure. Some research points to preterm delivery and lower birth weight with SSRI use across pregnancy, and depression carries those same associations, so the two are difficult to separate in the data.
- Third trimester. Fluoxetine has a long half-life, so a baby exposed near delivery may be irritable or jittery, cry more, have tight muscles, breathe with effort, sleep oddly, or struggle to eat. Which antidepressant is safest during pregnancy?

The medication that actually keeps you well still counts heavily in that calculation, which is why the choice belongs with your clinicians.
Does fluoxetine cause heart defects?
A Bayesian analysis found an elevated estimate for one specific heart defect, right ventricular outflow tract obstruction, at about twice background. The broader Medicaid cohort of 949,504 pregnancies found no substantial cardiac risk from antidepressants overall once depression was accounted for. Those two findings coexist, which is why this is a conversation and not a verdict.
Can I breastfeed while taking Prozac?
Fluoxetine does get into breast milk, and MotherToBaby reports that most published cases found no side effects in breastfed babies. Because fluoxetine lingers longer than some other SSRIs, ask your pediatric and obstetric providers what to watch for in your baby, especially in the first weeks.
