Is Mylanta safe during pregnancy?

Assigned clinical reviewerMatt Stahl· PharmDDraft · pending clinical reviewUpdated

What does the evidence say about mylanta during pregnancy?

  1. Mylanta Maximum Strength Classic Flavor lists aluminum hydroxide 800 mg, magnesium hydroxide 800 mg and simethicone 80 mg per 10 mL dose, with a cap of 60 mL (6 doses) in any 24-hour period and a warning not to use the maximum dosage for more than 2 weeks. 1
  2. Antacids work by neutralizing stomach acid and inhibiting pepsin. Antacids containing aluminum salts are safe to be used in pregnant women, while excessive use of calcium antacids should be avoided. 2
  3. The primary side effects of aluminum hydroxide include hypomagnesemia, hypophosphatemia, constipation and anemia, and prolonged use should not be considered in someone with renal impairment because impaired clearance of excess aluminum can precipitate those effects. 3
  4. The upper limit for magnesium in pregnancy is 350 mg a day and it applies only to supplemental magnesium, since magnesium found naturally in foods and beverages has no upper limit. The pregnancy RDA for magnesium is 350 to 400 mg a day depending on age. 4
  5. Antacids can help relieve mild symptoms but should not be used daily or for severe symptoms without talking to a doctor, and their side effects include diarrhea or constipation. 5

Is mylanta safe in each trimester?

  • First trimester. First-trimester reflux is less common but the ingredient logic already applies. If your main problem this early is nausea rather than acid, an antacid is aimed at the wrong target, and the aluminum component can add constipation to a system that iron is already slowing down.
  • Second trimester. This is where the magnesium ceiling becomes relevant. The upper limit of 350 mg a day applies to supplemental magnesium, and magnesium hydroxide in an antacid is supplemental magnesium. Six doses of 10 mL is 4,800 mg of magnesium hydroxide, so counting doses is not fussiness.
  • Third trimester. Late pregnancy is when people quietly move to the maximum dose daily. The label caps it: no more than 60 mL in any 24-hour period and no more than 2 weeks at the maximum dosage. Iron timing also matters now, since antacids reduce the absorption of several drugs.

What each Mylanta Maximum Strength active does

Active ingredientAmount per 10 mLLabel purposeWhat to know
Aluminum hydroxide800 mgAntacidSide effects include constipation, hypomagnesemia, hypophosphatemia and anemia. StatPearls states aluminum-salt antacids are safe in pregnancy.
Magnesium hydroxide800 mgAntacidCounterbalances the aluminum. The 350 mg daily upper limit in pregnancy applies to supplemental magnesium, which this is.
Simethicone80 mgAnti-gasAimed at pressure and bloating rather than acid, which is why the label covers both symptom sets.

Frequently asked questions

What is the difference between Mylanta and Tums?

Different metals. Tums is calcium carbonate. Mylanta Maximum Strength pairs aluminum hydroxide with magnesium hydroxide and adds simethicone for gas. That changes the side-effect profile, since aluminum and calcium push toward constipation while magnesium pushes the other way.

Does the aluminum in antacids cause constipation?

Constipation is listed among the primary side effects of aluminum hydroxide, alongside hypomagnesemia, hypophosphatemia and anemia. That is exactly why it is formulated with magnesium hydroxide, which pulls in the opposite direction. The balance works better for some people than others.

Can Mylanta affect how much iron you absorb?

Antacids reduce the absorption of a number of drugs. StatPearls gives specific guidance for fluoroquinolone antibiotics, where the effect is greatly mitigated if aluminum hydroxide is given at least two hours after the antimicrobial. Spacing your prenatal from your antacid follows the same logic.

How often can you take Mylanta in a day?

The label caps it at 60 mL, six doses, in any 24-hour period, and says not to use the maximum dosage for more than 2 weeks. NIDDK adds that antacids are not meant for daily use or for severe symptoms without a doctor's input.

Who should ask a doctor before using Mylanta?

The label names kidney disease and a magnesium-restricted diet specifically. Both matter because this product delivers real amounts of two minerals, and impaired clearance is what turns aluminum accumulation from theoretical into clinical.
