1st trimester

The first trimester is when ondansetron is most often started and when the collision happens: severe nausea means less food and less fluid, iron is in the prenatal, and the medication adds its own effect. Fewer than three bowel movements a week is the working definition to measure against.

Weeks 14–27

2nd trimester

If nausea has eased, the second trimester is the moment to ask whether the medication is still needed, because stopping the cause is more effective than layering something on top. That is a conversation with your provider, not a solo taper.

Weeks 28–birth

3rd trimester

Late pregnancy adds mechanical pressure to the pile. Polyethylene glycol is described as the drug of choice for chronic constipation in pregnant women, and osmotic laxatives are poorly absorbed by the intestine so very little is likely to reach the bloodstream.

What does the evidence show for constipation while taking ondansetron?

• Ondansetron is a selective 5-HT3 serotonin-receptor antagonist, and its most common side effects, occurring in more than 10 percent of adults, include headaches, fatigue, dry mouth, malaise and constipation. 1
• Most studies have not reported an increased chance of birth defects among thousands of women who used ondansetron in pregnancy, though a few studies reported a less than 1 percent increase in the chance of a cleft palate or a heart defect, findings other studies have not confirmed. 2
• Constipation means fewer than three bowel movements a week, with stools that are hard, dry or lumpy and difficult or painful to pass, and NIDDK names iron supplements, antacids and narcotic pain medicines among the medicines that can make constipation worse. 3
• Osmotic medications including polyethylene glycol are not well absorbed by the intestine, so very little is likely to get into the bloodstream, and fiber laxatives such as psyllium and methylcellulose generally do not enter the bloodstream either. Using laxatives as directed in pregnancy is not expected to have long-term effects on a baby's learning or behavior. 4
• Polyethylene glycol 3350 is dosed at 17 g mixed in 8 ounces of liquid once daily, with an initial response ranging from 2 to 4 days, and it is considered the drug of choice for chronic constipation in pregnant women. 5

When should I call my provider about constipation while taking ondansetron?

Constipation on ondansetron becomes urgent when it stops being just slow. NIDDK says to see a doctor right away if constipation comes with bleeding from your rectum, blood in your stool, constant pain in your abdomen, inability to pass gas, vomiting, fever, lower back pain, or losing weight without trying. The inability-to-pass-gas combination matters most here, because ondansetron slows gut motility and that pattern alongside abdominal pain and a distended belly is an obstruction picture rather than ordinary constipation. Vomiting is the confusing one on this medication, since you may already be taking it for nausea: new vomiting that arrives after your nausea had settled, especially with abdominal pain and no bowel movement, is a call rather than a wait. Whatever the pattern, any symptom that feels beyond your usual range is a reason to call: obstetric teams expect these questions and would rather hear early than late.
