Cervidil induction: process, timing, and risks
What does the evidence say about cervidil induction during pregnancy?
- The current U.S. label specifies one 10-mg vaginal insert for up to 12 hours, releasing about 0.3 mg per hour. Cervidil is administered only by trained obstetric staff in a hospital with appropriate facilities. 1
- The label requires removal at active labor, 12 hours, before amniotomy, after membrane rupture, or with uterine tachysystole, hypertonicity, or fetal distress. It also requires removal at least 30 minutes before an oxytocic agent and contraindicates use after prior cesarean or major uterine surgery. 1
- A randomized trial of 336 term inductions compared the Cervidil insert with hospital-formulated prostaglandin E2 gel. Cervical-score change favored the insert, oxytocin use was lower, and labor duration and cesarean differences were not statistically significant. 2
- In a small randomized trial of 52 first-time mothers at 38 weeks or later, controlled-release dinoprostone pessary produced lower reported pain than repeated gel, while oxytocin and vaginal-birth rates were similar. The small selected sample limits broad prediction of pain. 3
- A double-blind trial of 1,358 women compared a misoprostol insert with a 10-mg dinoprostone insert. Misoprostol shortened time to delivery but caused more tachysystole requiring intervention, while cesarean rates were similar, so speed alone does not establish the better option for an individual. 4
Is cervidil induction safe in each trimester?
- First trimester. Cervidil's labeled labor-induction use does not apply in the first trimester. Questions about dinoprostone for another obstetric purpose require separate specialist guidance and should not use the term-induction schedule.
- Second trimester. The labeled Cervidil indication is for cervical ripening at or near term when induction is medically or obstetrically indicated. It is not a home method for starting contractions earlier in pregnancy.
- Third trimester. Near term, trained obstetric staff place the insert and monitor contractions, fetal status, and cervical change. The retrieval system lets staff remove it when labor begins or when uterine or fetal findings call for stopping the drug.

What does Cervidil do during an induction?
Cervidil releases dinoprostone, a prostaglandin analog, to ripen an unfavorable cervix before labor induction. It may also bring on contractions, so uterine activity, fetal heart rate, and cervical change are monitored while the insert is in place.
How is Cervidil placed and removed?
A trained obstetric clinician places the flat insert high in the vagina. It sits inside a knitted retrieval pouch with a tape-like end that remains accessible. Staff remove the entire insert when the labeled stopping point or a clinical reason occurs.
How long does a Cervidil induction take?
The label allows the insert to remain for up to 12 hours, but it may come out earlier. Total induction-to-birth time is not fixed because cervical readiness, parity, membrane status, response, and the need for oxytocin or another method differ.
Can Cervidil be removed if contractions are too frequent?
Yes. Removability is a key feature of the insert. The label directs staff to remove Cervidil for uterine tachysystole, excessive uterine tone, fetal distress, active labor, or other listed stopping conditions while monitoring continues.
When can oxytocin start after Cervidil?
The current U.S. label requires Cervidil removal at least 30 minutes before an oxytocic medicine is started. Concurrent intravenous oxytocic use is contraindicated because dinoprostone can augment uterine activity.
Can Cervidil be used after a prior C-section?
No under the current U.S. label. A history of cesarean birth or other major uterine surgery, such as myomectomy, is listed as a contraindication because prolonged uterine contraction can threaten uterine integrity.
Is Cervidil induction painful?
Cervidil can lead to cramping and contractions, and individual pain varies. A small trial found less reported pain with the controlled-release pessary than repeated dinoprostone gel, but 52 first-time mothers cannot predict one person's experience.
What risks are monitored during Cervidil induction?
The team watches for excessive or overly frequent contractions, fetal heart-rate concerns, and labor progress. The label also lists rare serious events and directs immediate removal for persistent tachysystole or fetal distress, which is why Cervidil is limited to hospital use.
