Is berberine safe during pregnancy?

Assigned clinical reviewerMatt Stahl· PharmDDraft · pending clinical reviewUpdated

What does the evidence say about berberine during pregnancy?

  1. The main pregnancy-exposure data comes from one study of 218 pregnancies exposed to a berberine-containing herb, which found a small number of nervous-system and genital defects; it is not known whether berberine itself increases the chance of birth defects. 1
  2. Berberine passes into breastmilk in unknown amounts and can change how bilirubin binds to protein in the blood, which may raise a nursing baby's risk of bilirubin buildup in the brain. 1
  3. NIH's National Center for Complementary and Integrative Health states berberine is likely unsafe for infants and may also be unsafe during pregnancy or while breastfeeding because of possible effects on the fetus or infant. 2
  4. Berberine is being studied in women with PCOS-related infertility for its insulin-sensitizing effects, including activation of the AMPK pathway, but the same review notes evidence specific to use during an ongoing pregnancy remains limited. 3
  5. Berberine taken by mouth is poorly absorbed, with well under 1% reaching circulation in animal studies because of extensive first-pass metabolism in the gut and liver, which is part of why studied doses vary so widely between products. 4

Is berberine safe in each trimester?

  • First trimester. Human safety data is thin: the main study MotherToBaby cites followed pregnancies exposed to a berberine-containing herb and found a small number of nervous-system and genital defects, but it isn't known whether berberine itself raises the overall chance of birth defects.
  • Second trimester. No pregnancy-specific dosing or safety studies exist for this stage either. The insulin-sensitizing effects researched for PCOS and fertility haven't been tested for continued use once pregnant.
  • Third trimester. The bilirubin-displacement risk is most relevant close to delivery, since a newborn exposed to berberine, including through breastmilk afterward, may be more likely to develop jaundice severe enough to affect the brain.

Frequently asked questions

Is berberine safe during pregnancy and breastfeeding?

No major health authority recommends it. NCCIH calls it likely unsafe in both pregnancy and breastfeeding, largely because of the newborn bilirubin risk, and MotherToBaby notes there isn't enough human data to rule out a birth-defect risk either.

Can berberine cause miscarriage?

It isn't known. The pregnancy-exposure data on berberine-containing herbs is limited to one small study, and no research has directly measured miscarriage risk from berberine supplements specifically.

Does berberine help with weight loss like Ozempic?

Not to the same degree. NCCIH points to a 2022 review that found modest weight and BMI reductions, but only at doses above 1 gram a day for more than 8 weeks, and many of the underlying studies had a high risk of bias. That's a much thinner evidence base than for prescription GLP-1 medications, and none of it was studied in pregnancy.

Can you take berberine while trying to get pregnant?

Berberine is researched for PCOS-related infertility because of how it improves insulin sensitivity, but once you're actively trying or get a positive test, most guidance is to stop, since there's no established safety window for an ongoing pregnancy. Talk with your provider about timing.
