Is 5-HTP safe during pregnancy?

What does the evidence say about 5-HTP during pregnancy?

  1. NCCIH's review of sleep supplements notes that 5-HTP has a sedative effect but that it's unclear how it works and there is very limited data on its effects on insomnia, and it flags that 5-HTP has been kept under close safety watch because of its chemical relationship to L-tryptophan, a related supplement pulled from the market in 1989 after an outbreak of a rare, potentially fatal disorder called eosinophilia-myalgia syndrome. 1
  2. 5-HTP crosses the blood-brain barrier freely and, unlike tryptophan, isn't regulated by the body's normal feedback mechanisms, which is part of why combining it with a serotonin-affecting medication can push serotonin to dangerous levels: a case report describes a man who developed serotonin syndrome and severe muscle breakdown after combining a 5-HTP supplement with the antidepressant sertraline, an interaction serious enough to require emergency surgery. 2
  3. Increasing serotonin synthesis is one recognized pathway to serotonin toxicity. Combining substances that affect serotonin warrants particular caution, so a 5-HTP supplement should be included in a medication and supplement review. 3
  4. Laboratory research at the human maternal-fetal interface describes mechanisms involved in placental serotonin handling. This is biological context, not evidence that a particular 5-HTP supplement dose is safe or harmful in human pregnancy. 4
  5. In an animal study, mice exposed to serotonin before pregnancy went on to have measurably lighter placentas, more resorbed embryos, and lighter-than-expected offspring at two weeks old, an early signal that maternal serotonin levels can affect fetal growth, even though this hasn't been studied directly with 5-HTP supplementation in humans. 5

Is 5-HTP safe in each trimester?

  • First trimester. 5-HTP converts into serotonin and is built to cross the blood-brain barrier, so first-trimester exposure would be expected to reach the developing baby directly. There's no dedicated human safety research at any stage of pregnancy to point to a lower-risk dose or timing.
  • Second trimester. The same lack of pregnancy-specific safety data continues through the second trimester. If you're using 5-HTP for sleep or mood, that's a conversation to have with your prenatal provider rather than something to continue on your own.
  • Third trimester. Animal research on maternal serotonin exposure has linked it to reduced placental weight and slower fetal growth, though that evidence comes from rodent studies, not humans. There's no established guidance on stopping 5-HTP before delivery either, another reason to involve your provider rather than self-manage the timing.

Frequently asked questions

Can you take 5-HTP while pregnant for sleep?

There's very limited data on 5-HTP for sleep even outside of pregnancy. NCCIH notes it has a sedative effect, but how well it works for insomnia and how it works aren't well established, and no research has looked at using it for sleep specifically during pregnancy. Ask your provider about pregnancy-appropriate options instead.

Is 5-HTP safe during pregnancy and breastfeeding?

The cited evidence does not establish a safe 5-HTP supplement dose for pregnancy or breastfeeding. Crossing the blood-brain barrier does not by itself quantify placental transfer or breast-milk exposure. Ask your provider about the reason you want to use 5-HTP and options with better pregnancy or lactation evidence.

Can 5-HTP cause serotonin syndrome?

A published case report describes serotonin syndrome after 5-HTP was used with sertraline, with severe muscle injury and other complications. The report establishes a reason for caution but does not measure the frequency of this interaction in pregnancy. Have a pharmacist or prescriber review serotonergic medicines and supplements together.

Can I take 5-HTP if I'm also taking an antidepressant during pregnancy?

This combination needs your provider's input, not a decision made on your own. Many people continue an SSRI or SNRI during pregnancy under medical guidance, and adding a serotonin-boosting supplement on top of that medication raises real serotonin syndrome risk. Tell your provider about every supplement you're taking alongside a prescription.

What is 5-HTP and how is it different from melatonin?

5-HTP comes from tryptophan and is the direct chemical step before the body makes serotonin, which can be converted further into melatonin. That's a more roundabout path than taking melatonin directly, which is a separate, more commonly studied supplement with its own pregnancy safety conversation.
