1. The June 2026 Wegovy label directs stopping for weight or cardiovascular-risk indications when pregnancy is recognized and planning at least a two-month interval before conception. Its narrow MASH indication requires a specialist benefit-risk decision. Breastfeeding is not recommended with Wegovy tablets because of the SNAC absorption enhancer. 1
2. MotherToBaby reports that most semaglutide can take up to about six weeks to leave the body and that product labels recommend a two-month interval before pregnancy. It summarizes limited human reports, including 168 pregnancies exposed to a GLP-1 medicine, only 51 of them semaglutide. 2
3. A prospective cohort compared 168 first-trimester GLP-1 receptor agonist exposures with 156 pregnancies affected by diabetes and 163 pregnancies affected by overweight or obesity. Is wegovy safe in each trimester?

• First trimester. Most inadvertent exposure occurs before pregnancy is recognized. Contact the prescriber promptly and identify why Wegovy was prescribed. The current label says to discontinue it when pregnancy is recognized for weight or cardiovascular-risk indications, while the available human cohorts are too small and mixed to rule out uncommon outcomes.
• Second trimester. Weight reduction offers no benefit during pregnancy according to the label. If Wegovy was being used in the setting of diabetes, obesity-related risk, or metabolic dysfunction-associated steatohepatitis, the obstetric and prescribing teams should plan ongoing nutrition, weight, liver, and glucose care without assuming that stopping resolves the underlying condition.
• Third trimester. Do not restart Wegovy for pregnancy weight control. Review glucose and fetal-growth monitoring when relevant, and plan postpartum feeding with the prescriber. Wegovy tablets carry a recommendation against breastfeeding because of SNAC. Limited low-dose injectable semaglutide milk findings do not establish safety for higher doses or exclusively breastfed newborns.
