Is tilapia safe during pregnancy?

Assigned clinical reviewerMichael Yuzefovich· MD, FACOGDraft · pending clinical reviewUpdated

What does the evidence say about tilapia during pregnancy?

  1. Tilapia sits in the Best Choices category, where the guidance for pregnancy is 2 to 3 servings a week, with one serving defined as 4 ounces. 1
  2. Tilapia averages 0.013 parts per million of mercury in FDA sampling, placing it near the very bottom of a table sorted from lowest to highest. 2
  3. Eating one or more servings of fish per week during pregnancy was linked to better offspring neurodevelopment than eating none in seven of eight studies reviewed, and reviewers concluded the two to three servings per week recommendation looks appropriate. 3
  4. Fish is done when the flesh is clear and separates easily with a fork, and most seafood should reach an internal temperature of 145°F. 4
  5. Tilapia is an occasional source of histamine (scombroid) illness despite being a light-meat fish, with surveillance finding levels up to 290 mg per 100 g when clinical toxicity starts around 50 mg per 100 g, which is why unbroken cold storage at or below 40°F matters. 5

Is tilapia safe in each trimester?

  • First trimester. Tilapia is an easy first-trimester fish because it is mild, cheap, and cooks in minutes, which matters when smells are setting off nausea. There is no lower limit to work around: the 2 to 3 servings a week guidance applies from the start. If fish is off the menu entirely right now, that is common and worth mentioning at your first visit.
  • Second trimester. Appetite usually returns in the second trimester, and this is a good stretch to build the fish habit rather than catch up later. Rotate tilapia with other Best Choices instead of eating the same fish every time. Baked or pan-seared keeps it simple, and leftovers should go in the fridge quickly rather than sitting out.
  • Third trimester. Your baby's brain and eye tissue take up DHA fastest in the third trimester, so steady fish intake earns its place now. Tilapia is lean, so it is a lower-DHA option than salmon or sardines. Pairing it across the week with an oily Best Choice covers both the low-mercury and the omega-3 side.

Frequently asked questions

How much tilapia can I eat while pregnant?

Two to three servings a week, where a serving is 4 ounces weighed before cooking. That is the same allowance given to every fish in the Best Choices group. Mixing tilapia with other low-mercury options across the week is better than eating tilapia every time.

Is tilapia high in mercury?

No. FDA sampling puts tilapia at an average of 0.013 parts per million, one of the lowest figures in the whole table. For comparison, swordfish averages 0.995. Mercury is simply not the concern with this fish.

Is farmed tilapia safe during pregnancy?

FDA's advice chart does not split tilapia into farmed and wild, and the mercury average comes from sampling of tilapia generally. What matters more for any tilapia is that it stayed cold from the store to your pan and that you cook it through.

How do I know when tilapia is cooked through?

The flesh turns from translucent to clear and flakes apart easily when you press a fork into the thickest part. FDA's number is 145°F on a thermometer. Tilapia fillets are thin, so they get there fast and dry out if you push much past it.
