What does the evidence say about sumatriptan during pregnancy?

Is sumatriptan safe in each trimester?

Acetaminophen is still the usual first step, with sumatriptan held for attacks it does not reach.
  • Second trimester. Migraine often eases in the second trimester as hormones settle, so this is a natural point to review how many attacks you are actually having and how often you are reaching for a triptan. Frequency matters more than any single dose when your clinician is weighing options.
  • Third trimester. Later triptan use has been linked to a modest rise in preterm delivery in some research, an outcome also tied to migraine itself, and one large cohort found associations with atonic uterus and heavier blood loss during labor. Any severe new headache this late needs same-day evaluation, since it can signal preeclampsia.
