What does the evidence say about Mucinex during pregnancy?

1. Guaifenesin, the only active ingredient in plain Mucinex, carries at most a low added chance of birth defects if any, and it's best to choose an alcohol-free product during pregnancy. 1

The birth-defect concern with pseudoephedrine is specific to the first trimester, so by now plain guaifenesin, dextromethorphan, or even Mucinex D can be reasonable options once your provider signs off. Still avoid alcohol-containing liquid formulas, and check labels closely if you're also taking Tylenol, since Fast-Max already includes acetaminophen.
• Third trimester. Guaifenesin and dextromethorphan remain reasonable options in the third trimester when your provider directs their use, and it's still worth reading the active-ingredient panel. Mention any Mucinex D or Fast-Max use to your obstetric team, since these can shift blood pressure and fluid balance late in pregnancy.
