What does the evidence say about lifting at work during pregnancy?
- Across 68,086 working women in the Danish National Birth Cohort, first-time mothers lifting 201 to 975 kilograms a day had higher odds of preterm birth (hazard ratio 1.43), with a clear dose-response trend across load levels. 1
- In that same cohort, no exposure-response relationship with fetal death appeared across the full population, but among the 12,131 women with a previous fetal loss, lifting 201 to 975 kilograms a day was associated with a hazard ratio of 2.87 for stillbirth. 1
- Among 47,582 Danish pregnancies, the average daily lifting burden was 186 kilograms among women who lifted at all, and associations with birth size weakened once women lifting over 1,000 kilograms a day were excluded. 2
- A 2023 systematic review rated the evidence tying heavy lifting alone to preterm birth as very low, with no statistically significant association, while finding moderate evidence that overall high physical workload is associated with higher risk. 3
- Among nurses, lifting heavy loads 16 or more times a day was linked to preterm birth in the simpler model (relative risk 1.5) but the association faded once other job factors were accounted for (relative risk 1.2, confidence interval crossing 1). 4
Is lifting at work safe in each trimester?
- First trimester. This is when to renegotiate the job rather than tough it out, because a lighter routine set now runs for the whole pregnancy. In one Danish cohort, women with a previous pregnancy loss showed the strongest signal at high daily loads.
- Second trimester. Your center of gravity has moved forward and your abdominal muscles are being stretched, so the mechanics of a lift you have done for years are no longer the same. Push and pull rather than carry where you can.
- Third trimester. Bending is now the harder half of the movement. In one UK cohort, bending the trunk more than an hour a day was linked to preterm delivery at 34 weeks, so raising things to waist height is worth the setup time.

How much weight can I lift at work while pregnant?
There is no single number that fits every job, and the research measures daily totals rather than one-off maximums. Signals start appearing above roughly 200 kilograms a day. Your clinician can set a specific limit for your role and your history.
Can heavy lifting cause a miscarriage?
In the largest Danish cohort no relationship with fetal death appeared across the whole population. A signal did show up in women who had already had a pregnancy loss, which is why prior history is the thing to raise with your clinician.
What is the safest way to lift while pregnant?
Cut the number of lifts before you refine the technique, since daily total is what the studies track. When you do lift, keep the load close, avoid twisting, and raise things to waist height so you bend less.
Should I get a lifting restriction from my doctor?
It is worth asking, particularly if your job involves repeated lifting, you have had a previous preterm birth or loss, or you are in the third trimester. A written restriction gives your employer something concrete to work with.
