Is Erythromycin Safe to Take During Pregnancy?
What does the evidence say about erythromycin during pregnancy?
- MotherToBaby concludes that most studies do not show a higher chance of birth differences with erythromycin. A few studies suggested heart differences or pyloric stenosis, but those findings were not consistently confirmed. 1
- Current CDC chlamydia guidance recommends azithromycin in pregnancy and lists amoxicillin as an alternative. Erythromycin is no longer recommended because gastrointestinal effects can make adherence difficult and because cautious use is advised. 2

Is erythromycin safe in each trimester?
- First trimester. Most first-trimester studies do not show an overall rise in major birth differences, but a few signals have not been fully resolved. Use the exact formulation and course selected by the prescriber for the infection being managed.
- Second trimester. Later-pregnancy evidence has not shown a consistent rise in preterm birth or low birth weight. Finishing a prescribed course matters, but contact the prescriber if nausea, vomiting, or diarrhea makes doses hard to keep down.
