• Second trimester. Bowel cramping often eases in the second trimester as hormones settle, so this can be a natural point to revisit whether you still need an antispasmodic at all. Diet changes, soluble fiber and stress work carry no fetal question mark and are the usual starting place.
• Third trimester. Dicyclomine dries secretions and slows the gut, which can make late-pregnancy constipation and reflux worse rather than better. It is also contraindicated once you are nursing, so it is worth planning the handoff to something else before delivery rather than after.
