P5P vs pyridoxine B6 for morning sickness

Assigned clinical reviewerMatt Stahl· PharmDDraft · pending clinical reviewUpdated

How do the options compare?

Options are compared on ACOG or MotherToBaby endorsement, size and quality of pregnancy-specific studies, dosing predictability, time-to-effect, and side-effect profile: the same axes an obstetric clinician would use.

OptionStudied pregnancy doseGuideline supportNotes
Pyridoxine hydrochloride (plain B6)10 mg per tablet in the delayed-release combination studied for nausea and vomiting of pregnancy. The pregnancy RDA is 1.9 mg a day, against a 100 mg adult upper limitThis is the form the trials used, and the combination of doxylamine succinate with pyridoxine hydrochloride has been studied in hundreds of thousands of pregnancies without an increased chance of birth defectsThe most common supplement form. Chronic 1 to 6 g a day for 12 to 40 months has caused severe progressive sensory neuropathy, though symptoms usually stop when the supplement is discontinued
P5P (pyridoxal 5'-phosphate)No pregnancy-specific studied dose. The 100 mg adult upper limit for vitamin B6 covers every formPLP is the biologically active coenzyme form of vitamin B6, and some supplements contain PLP rather than pyridoxineA secondary analysis linked higher serum PLP to nausea resolving sooner: the highest-PLP group had a 46% lower risk of persistent symptoms (HR 0.54), with a linear dose-response between PLP and time to remission. That is an observation about blood levels, not a trial of the P5P capsule
  • Pyridoxal 5' phosphate (PLP) is the active coenzyme form of vitamin B6, while most supplements contain pyridoxine hydrochloride and only some contain PLP. 1
  • The delayed-release combination tested in the randomized placebo-controlled trial for nausea and vomiting of pregnancy used pyridoxine 10 mg per tablet alongside doxylamine 10 mg, taken as 2 to 4 tablets daily. 2
  • In a secondary analysis, participants with the highest serum PLP levels had a 46% lower risk of persistent nausea and vomiting (HR 0.54), with a linear dose-response between PLP and time to remission. 3
  • The prescription delayed-release product lists pyridoxine hydrochloride 10 mg per tablet, not pyridoxal 5'-phosphate, so the pharmacy version of this pairing is the plain B6 form. 4
  • Doxylamine succinate paired with pyridoxine hydrochloride has been studied in hundreds of thousands of pregnancies without an increased chance of birth defects. 5

Which option makes sense?

A comparison can include options that should be avoided or considered only in specific circumstances. Please consult your healthcare provider about the findings and cautions that apply to you.

Disclosure: Pregnancy Smart makes supplements that overlap with some options above. This comparison describes the evidence, not our own product; where our formulation is relevant it is named.

Frequently asked questions

Is P5P better absorbed than pyridoxine?

The NIH fact sheet describes PLP as the active coenzyme form, which is where the better-absorbed marketing comes from. It does not compare absorption of the two supplement forms head to head, and neither does any pregnancy trial we could source. Better-studied and better-absorbed are not the same claim.

What form of B6 is in Diclegis?

Pyridoxine hydrochloride, 10 mg per delayed-release tablet, paired with 10 mg of doxylamine succinate. If your provider suggested the studied combination, that is the form they meant.

Is P5P safe during pregnancy?

There is no pregnancy-specific safety or dosing study for the P5P form that we could source. The 100 mg adult upper limit for vitamin B6 applies to all forms, so the sensible reading is that P5P is not established rather than established as risky. Bring the bottle to your provider.

How much P5P equals 25 mg of pyridoxine?

No published equivalence exists in the sources on this page, and we will not invent one. Milligrams of P5P and milligrams of pyridoxine hydrochloride are not interchangeable numbers, which is exactly why a pharmacist is the right person to ask before you swap bottles.

Does the B6 in my prenatal count toward the total?

Yes. The pregnancy RDA is 1.9 mg a day and the adult upper limit is 100 mg. Your prenatal, any standalone B6 tablet, and a doxylamine-plus-B6 regimen all add into the same daily total, so read all three labels before stacking.
