Daily vs every-other-day iron in pregnancy
How do the options compare?
Options are stacked on five dimensions relevant to pregnancy use: mainstream OB guidance, pregnancy-specific evidence, dosing predictability, onset, and side-effect and interaction profile.
| Option | Studied pregnancy dose | Guideline support | Notes |
|---|---|---|---|
| Daily iron | One ferrous sulphate tablet with 60 mg elemental iron daily in the randomized trial. The pregnancy RDA is 27 mg a day and the adult upper limit is 45 mg | Consensus work reports that maximal fractional absorption of iron occurred at 60 mg of elemental iron, and that consensus is lacking on the optimal dose in iron deficiency without anaemia | Oral iron acutely raises serum hepcidin and the rise persists for 24 hours, which reduces absorption from a dose given the following day. Hemoglobin rose 1.36 g/dL in the daily arm |
| Every-other-day iron | Two ferrous sulphate tablets, 60 mg elemental iron each (120 mg total), taken on alternate days in the randomized trial | Intermittent dosing overcomes the hepcidin suppression, and alternate-day oral dosing can be considered to limit side effects in women with obstetric iron deficiency | Fractional absorption ran 40 to 50% higher than consecutive-day dosing, and total absorption from 200 mg on alternate days was roughly twice that from 100 mg on consecutive days. Hemoglobin rose 1.05 g/dL, and nausea was reported more often at week 4 (10.3% versus 2%) |
- Oral iron doses of 100 to 200 mg acutely raise serum hepcidin and the increase persists for 24 hours, which is the mechanism behind spacing doses out. 1
- Fractional iron absorption on alternate-day dosing ran 40 to 50% higher than on consecutive-day dosing, and total absorption from a single 200 mg dose on alternate days was roughly twice that from 100 mg on consecutive days. 1
- In a randomized controlled trial, mean hemoglobin rose 1.05 g/dL on alternate-day iron against 1.36 g/dL on daily iron, a difference that was not statistically significant, while nausea was reported more often in the alternate-day arm at week 4 (10.3% versus 2%). 2
- A consensus statement on iron in pregnancy notes that maximal fractional absorption occurred at 60 mg of elemental iron, that intermittent dosing overcomes hepcidin suppression, and that alternate-day dosing can be considered to limit side effects in obstetric iron deficiency. 3
- Iron sits at 27 mg a day for pregnancy against a 45 mg adult upper limit, high-dose supplements bring constipation, nausea, and gastric upset, ascorbic acid improves nonheme iron absorption, and calcium might interfere with it. 4
Which option makes sense?
Options in this comparison can differ substantially in suitability. Please consult your healthcare provider to interpret the evidence, limitations, and cautions before choosing an option or changing your plan.

Frequently asked questions
Is it better to take iron every other day?
Better per dose, not clearly better per month. Spacing doses gets around the 24-hour hepcidin rise, so more of each tablet is absorbed. The randomized trial that measured the end result found no significant difference in hemoglobin. The honest answer is that it is a reasonable option, not a proven upgrade.
Does alternate day iron absorb better?
Yes, and the numbers are specific. Fractional absorption was 40 to 50% higher on alternate days than on consecutive days, and total absorbed iron from 200 mg every other day was about double that from 100 mg on consecutive days.
Does spacing out iron reduce constipation?
That is the usual reason for trying it, and consensus guidance says alternate-day dosing can be considered to limit side effects in obstetric iron deficiency. The same guidance is candid that evidence for better tolerability is insufficient, and one trial actually found more nausea in the alternate-day arm.
How long does it take to bring iron levels up in pregnancy?
The randomized trial measured hemoglobin at 4 weeks, with rises of about 1.0 to 1.4 g/dL in both arms. That is the realistic timescale for a repeat blood count, not days. Your provider sets the recheck interval based on your starting numbers.
Should you take iron with vitamin C?
It helps. The NIH fact sheet notes ascorbic acid enhances the bioavailability of nonheme iron, which is the type in most supplements. The same fact sheet notes calcium might interfere with iron absorption, and that individual calcium and iron supplements are best taken at different times of day.
